Care Transitions webinar

The MHA Keystone Center will be hosting a care transitions webinar from 2 to 3 p.m. Jan. 21. This webinar will provide an overview of the rehospitalization reduction project framework, process, and outcomes at Oaklawn Hospital. Successful strategies will be discussed including the development of a computerized plan of care for readmitted patients, simplification of discharge instructions, and utilizing emergency department case management.
