Care Transitions Webinar

The MHA Keystone Center will be hosting a care transitions webinar from 2 to 3 p.m. Oct. 15. on the INTERACT (Interventions to Reduce Acute Care Transfers) program. This free evidence-based program is being used in nursing homes across the country to reduce avoidable rehospitalizations and improve care transitions. This webinar is important for hospitals to understand the process and communication tools Skilled Nursing Facilities (SNFs) are currently using to prevent unnecessary hospitalizations.
