2014 Care Transitions Statewide Summit

The MHA Keystone Center and MPRO, Michigan’s Quality Improvement Organization, are pleased to announce the third annual Care Transitions Statewide Summit from 8:30 a.m. to 4 p.m. May 28 at the Kellogg Hotel & Conference Center, East Lansing. 

The summit will bring together more than 300 participants from all facets of the healthcare industry and across the continuum of care to share care transition best practices, keep up-to-date on new strategies and connect with health professionals and providers. Keynote speakers for this event include Allison Massari, MFA, international motivational speaker, and Cheri Lattimer, RN, BSN, president & chief executive officer at Consulting Management Innovators, and coalition director of the National Transitions of Care Coalition.
