workshop registration
Registration deadline is Feb. 20, 2011. Space is limited and registrations after Feb. 20 will be taken on a first-come, first-served basis. On-site registrations will be accepted, room providing. Registrants who cannot make it the day of the workshop may send an alternate. Fax this registration form to 517-703-0616. Registration confirmation will be sent via email. If you have not received your confirmation within one week of the workshop, contact Ron Hubble at 517-886-8280 or rhubble@mha.org.
Upon registration please submit payment to the MHA Keystone Center. If your hospital wants to pay with a credit card, please provide credit card information on the second page of this registration form. The MHA Keystone Center staff will bill the credit card. If you do not provide credit card information on the attached registration form, please make checks payable to MHA Keystone Center and send checks to MHA Keystone Center, 6215 W. St. Joseph Highway, Lansing, MI 48917.
Cancellation policy: If notice of cancellation is given 72 hours in advance of the workshop, 50 percent of the registration fee is refundable. No refunds will be issued after this time. For a registration form, please contact Megan Black at mblack@mha.org
