Care Transitions webinar

The next care transitions webinar is on improving care coordination and collaboration with community partners from 2 to 3 p.m. Dec. 17. This webinar will feature the Washtenaw Health Initiative, and their work to improve care coordination in their community by supporting care and case managers from many systems to interact and work together.
