Join Us: MHA Keystone Center Age-Friendly Action Community
Enrollment is open for the MHA Keystone Center Age-Friendly Health Systems Action Community. This initiative, funded by the Michigan Health Endowment Fund, will bring attention to the 65 years and older population as well as demonstrate how to support the needs of older adults and provide high-quality care and better health outcomes.
What is an Action Community?
The MHA Keystone Center Age-Friendly Health Action Community will continue to build on the success of the national Age-Friendly Health Systems initiative, which was originally launched in 2017 by The John A. Hartford Foundation and Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association and Catholic Health Association of the United States. The Action Community is a group of teams across different health systems and settings that come together to accelerate the testing and adoption of the 4Ms Framework. Who can enroll?
- Hospital-based teams (e.g., emergency departments, ICUs, general wards, medical-surgical units).
- Ambulatory care teams (e.g., primary care, specialty care).
- Extended care teams (long-term care/skilled nursing facilities).
When does the Action Community Begin?
The Action Community begins in March and runs through September 2021. Two informational webinars will be held Dec. 15 and Jan. 21 to discuss the program requirements and overall framework. The content of both webinars is identical so that members can choose the most convenient date.
