Registration is open for the MHA Keystone Center Age-Friendly Health Systems Action Community Virtual Workshop: Accelerating the Adoption of the 4Ms. The two-day virtual event will be held May 18 & May 19 from 8:30 to noon both days.
This interactive virtual workshop will be composed of breakout sessions, deep-dives and action planning sessions. Participants will have the opportunity to meet others in the Age-Friendly movement, learn and problem-solve directly with peers and colleagues, and accelerate the adoption of the 4Ms (What Matters, Mentation, Medication and Mobility) through the sharing of supporting practices and tools guided by subject matter experts.
Storyboards – May 18
On May 18, participants will engage in a Storyboard Sharing Session. To prepare for the interactive portion, each facility will submit a short storyboard beforehand. The storyboard will include information on 4Ms progress, qualitative and quantitative results as well as questions and areas that participants wish to learn more about.
Each registered facility will be contacted within seven days of registration to receive the storyboard template. One storyboard will be shared per facility.
4Ms Breakout Sessions – May 19
Facilitated by subject matter experts, participants will engage in two 45-minute breakout sessions discussing approaches and tools to build on efforts within the 4Ms: What Matters, Medication, Mentation, and Mobility. Participants will collaborate with other teams and develop an action plan to take back to their facility.
During registration, each participant will select two of the “Ms” to engage in during breakout discussions. If your facility is registering multiple team members, please consider dividing your team members accordingly to cover all the 4Ms during the breakout session.
Who Should Attend?
All MHA Keystone Center Age-Friendly Action Community Cohort 2 participants are encouraged to attend both days of the virtual workshop. The event will include interactive deep-dive and planning sessions into the 4Ms; therefore, we strongly recommend registering a multidisciplinary team.
