  • Join the Age-Friendly Health Systems Action Community

    The MHA Keystone Center is currently recruiting hospitals for its Age-Friendly Health Systems Action Community. An invitation guide is available that provides additional information on the initiative.

    Important Information 
    • Funded by the Michigan Health Endowment Fund, the Age-Friendly Health Systems Action Community will be held from October 2019 to April 2020 for hospital-based teams (i.e. emergency departments, intensive care units, medical/surgical units, etc.) and ambulatory care teams (i.e. primary/specialty care).
    • Healthcare teams who are interested in participating can enroll online.
    • The MHA Keystone Center recently hosted two recruitment webinars. Recordings are available on the MHA Community site.
    • Michigan hospitals will be educated on the 4Ms framework: What Matters, Medication, Mentation and Mobility by faculty experts.
    • Additional educational opportunities include: site visits by MHA Keystone Center staff and subject matter experts (SME), monthly peer-coaching calls, monthly content webinars to share progress and monthly leadership and sponsor calls for board-level and C-suite members.
    • As an Age-Friendly Health Systems Action Community facilitator, the MHA Keystone Center will collect and report data on hospital team performance, and work with teams to identify areas of opportunities around the 4M framework implementation.
    • Teams will receive continuous support from the MHA Keystone Center, Institute for Healthcare Improvement and SMEs.
