  • New Effort Launched for Readmissions and Falls Improvement

    The Great Lakes Partners for Patients (GLPP) Hospital Improvement Innovation Network (HIIN) is working to achieve the bold aims set forth by the Centers for Medicare & Medicaid Services to reduce patient falls with injury by 20% and readmissions by 12%.

    To support these efforts, the GLPP HIIN is launching two Improvement Sprints designed to amplify the impact of your facility’s efforts within readmissions and falls. The Improvement Sprints are rapid implementation programs that will address project drift, provide technical and educational resources, and support the deployment of best practices.
